SPORTS PHYSICALS

REMINDER: Sports physicals are due before practice for sports begins. Please plan accordingly. Remember that the doctors and our nurse practitioner are occasionally out of the office during the summer months and if you wait too long to schedule an appointment we may not be able to accommodate you.

Generally, a physical is good for two years. If your child has had one within that time period you can drop the forms off and have them signed. Please note that it may take two BUSINESS DAYS to return the forms. This means if you drop it off on a Friday it may not be done until Tuesday of the following week.

The green WIAA forms have a portion that must be completed by the parent and the athlete. This includes a lengthy questionnaire.  It needs to be filled out prior to your appointment or prior to leaving it here to be signed. WE WILL NOT SIGN ANY PAPERWORK WITHOUT YOUR PORTION BEING FILLED OUT BY YOU!

Safe Care Wisconsin

Safe Care Wisconsin is a group of medical professionals and organizations.  They are launching a statewide program called List it. Don’t Risk it.  The program emphasizes the importance of patients writing down and carrying with them a list of the medications they take.  Important for everyone but most important for the elderly who may be on numerous medications and herbal remedies.  Having information like this in your wallet could save your life.  There is also a place on the form to list allergies.  Download a form today for you and a friend at www.safecarewisconsin.org and don’t forget to carry it with you at all times.
