Please plan accordingly. Remember that the doctors and our nurse practitioner are occasionally out of the office during the summer months and if you wait too long to schedule an appointment we may not be able to accommodate you. Generally, a physical is good for two years. If your child has had one within that time period you can drop the forms off and have them signed. Please note that it may take two BUSINESS DAYS to return the forms. This means if you drop it off on a Friday it may not be done until Tuesday of the following week. The green WIAA forms have a portion that must be completed by the parent and the athlete. This includes a lengthy questionnaire. It needs to be filled out prior to your appointment or prior to leaving it here to be signed. WE WILL NOT SIGN ANY PAPERWORK WITHOUT YOUR PORTION BEING FILLED OUT BY YOU!
