If you are in need of our services outside of office hours you may call the hospital designated by your insurance and ask the emergency room staff to contact us by pager. They will also know if we are "signed out" to other physicians in the community when we are out of town. You may reach us at home and we appreciate that this has been respected and used appropriately in the past. We will not respond to medical messages that are left on our home answering machine.
Schedule appointments as far in advance as possible. In order to provide the care you need, please provide as much information regarding the nature of your visit at the time of scheduling. This information will have an impact on the amount of time your visit will be scheduled for and with whom.
If you have orders from another healthcare provider to schedule an appointment within a specific timeframe, it is critical that you call to make the appointment as early as possible so that we can fill your request. If you call without much notice we may have difficulty fitting your request into our schedules as they are typically full.
Missed appointments increase the wait all patients experience in getting appointments. We understand that patients may occasionally forget an appointment. When this happens we record the missed appointment in your chart. If this happens more than once in a short time period, you will be charged $20.00 for these missed appointments. You will be personally responsible for this charge as it is not covered by insurance. This will also apply to any lab appointments that are missed.
Please let us know as soon as possible if you will be unable to be at an appointment. We request a minimum of 24 hour notice. We understand that last minute things occur; however it is unfair to other patients who are waiting as much as 3 months for appointments to have available appointment times wasted.
All lab appointments must be scheduled.
Plymouth Family Physicians is pleased to announce that we now provide early morning lab appointments. We do request that these be scheduled by our patients that are employed and have a difficult time getting in after 8:00 a.m. These appointments are held Tuesday and Thursday morning from 7:00 until 8:00 a.m. If you need one of the early appointments, please call to schedule.
Blood pressure checks are offered for free from 8 a.m. to noon and again from 1:30 p.m. to 4 p.m. daily. Please be prepared to wait at least 10 minutes after your arrival before your blood pressure is taken. You must be "at rest" at 10 minutes prior to the reading in order for the reading to be accurate. Also, a friendly reminder that our patients with appointments are serviced before walk-ins for blood pressure checks. We appreciate your patience and understanding in this regard.
Co-pays are required at the time of service. We accept cash, checks and credit cards. All patients should be prepared to provide our office with up-to-date insurance information at each visit.
If you need a refill for a prescription, you can call our office or follow this link to complete a request form on-line. We will attempt to fulfill all refill requests within 24 - 48 hours. If you are requesting a refill for high blood pressure, diabetes or a thyroid condition and are not up to date in your labs and/or appointments, we will only refill your prescription for 30 days. This is for your health and safety. Your cooperation is appreciated.
