Plymouth Family Physicians Receives Best Practice Award at National Meeting

PLYMOUTH FAMILY PHYSICIANS RECEIVES BEST PRACTICE AWARD AT NATIONAL MEETING

Seattle, Washington, July 23, 2005

At its annual meeting today, the Practice Partner Research Network (PPRNet) announced that Plymouth Family Physicians of Plymouth, WI, received a Best Practice award, signifying excellence in patient care across a broad spectrum of clinical conditions. PPRNet is a national primary care practice-based research and quality improvement organization, headquartered at the Department of Family Medicine, Medical University of South Carolina, located in Charleston, SC. PPRNet practices use an electronic medical record developed by Physician Micro Systems, Inc., Seattle WA. Participating practices pool anonymous clinical data for research and quality improvement activities. PPRNet is conducting a study funded by the U.S. Public Health Service Agency for Healthcare Research and Quality designed to accelerate translation of evidence-based medical guidelines into practices across the country. These guidelines are for prevention and treatment of heart disease and stroke, diabetes mellitus, cancer screening, immunizations, and respiratory disease, mental health and substance abuse, nutrition and obesity, and safe drug prescribing in the elderly. To receive a Best Practice award, Plymouth Family Physicians scored in the top 10% of PPRNet practices. SPORTS PHYSICALS

REMINDER: Sports physicals are due before practice for sports begins. Please plan accordingly. Remember that the doctors and our nurse practitioner are occasionally out of the office during the summer months and if you wait too long to schedule an appointment we may not be able to accommodate you.

Generally, a physical is good for two years. If your child has had one within that time period you can drop the forms off and have them signed. Please note that it may take two BUSINESS DAYS to return the forms. This means if you drop it off on a Friday it may not be done until Tuesday of the following week.

The green WIAA forms have a portion that must be completed by the parent and the athlete. This includes a lengthy questionnaire. It needs to be filled out prior to your appointment or prior to leaving it here to be signed. WE WILL NOT SIGN ANY PAPERWORK WITHOUT YOUR PORTION BEING FILLED OUT BY YOU!
